During registration, you will receive an SMS from 1414. To complete registration, you must reply to the message with "511".

Select a suitable organization from the list provided. The availability of the organization depends on the selected region and the availability of open appointment schedules.

At this step, you need to fill in the applicant's details. Provide the requested personal and contact information required to process the application. Verify the accuracy of the entered information before proceeding to the next step.

At this step, you need to provide information about the child for whom the service is being arranged. Fill in the requested data and verify its accuracy before proceeding to the next step.

At this step, you need to attach the documents required to obtain the service. Upload the necessary files to the corresponding fields and ensure that the documents are legible, up-to-date, and attached in the correct format. After uploading all documents, verify their presence and proceed to the next step.

At this step, you must verify the information provided in the application and sign it with an electronic digital signature (EDS). After successful signing, the application will be sent to the selected PMPC organization for consideration.
